  • 2 Forms of ID

  • Full Resume

  • Covid Vaccination (or Exemption)

  • Current BLS/CPR certification 

  • Current TB test/Chest X-ray 

  • Physical

  • Immunization (or Declination): (Measles, Mumps, Rubella, Varicella, Tetanus/Diphtheria, Hep B)

  • eSigns (these will sent to your email when you fill out the Kamana Profile Application)
